A 24-year-old male with blunt thoracic trauma presented with hemodynamic instability and dextrocardia on chest radiography, indicating pericardial rupture with cardiac herniation.
Case Report (n=1)
Highlights a rare presentation of pericardial rupture with cardiac herniation manifesting as dextrocardia on chest radiography following severe blunt thoracic trauma.
Traumatic dextrocardia may indicate cardiac herniation when FAST is limited; case reports leave open prospective validation of diagnostic strategies.
A previously healthy 24-year-old male patient presented to a rural hospital with hemodynamic and respiratory instability following a high-speed motor vehicle collision involving multiple fatalities. He was managed with aggressive intravenous fluid resuscitation, blood transfusions, bilateral chest tubes, and intubation. Chest radiography revealed dextrocardia, and a focused assessment with sonography for trauma examination was limited secondary to massive subcutaneous emphysema and the heart's abnormal anatomical location.

Leblanc et al. (2020) conducted a case report in Pericardial rupture with cardiac herniation following blunt thoracic trauma (n=1). Aggressive intravenous fluid resuscitation, blood transfusions, bilateral chest tubes, and intubation was evaluated. A 24-year-old male with blunt thoracic trauma presented with hemodynamic instability and dextrocardia on chest radiography, indicating pericardial rupture with cardiac herniation.
