Prominent R wave in lead aVR predicts ~28% EF and high VF risk in ALVC.
A prominent R wave in lead aVR may predict decreased left ventricular function and high risk of ventricular fibrillation in arrhythmogenic left ventricular cardiomyopathy.
May flag severe LV dysfunction and VF risk in ALVC; hypothesis-generating from single case and needs prospective validation.
A case of arrhythmogenic left ventricular cardiomyopathy was described. In the ECG of the patient prominant R wave in lead aVR was noted predicting a significantly descreased left ventricular function (EF 28%) and a high risk of clinical or inducible VF. An ICD was implanted for prophylactic reasons. Under GDMT the EF was increased to 40%.
