Granulocytic sarcoma (GS) is a rare disease corresponding to extramedullary tumor mostly associated with acute myelogenous leukemia. Despite the aggressiveness of leukemic cells, F-18-FDG PET is not recommended for leukemia but it may have relevance for these tumors with solid and malignant organization. Two patients presented with acute myelogenous leukemia and GS. Every known malignant site showed significant uptake of F-18-FDG and we described many unknown locations. For 1 patient, PET revealed an important metabolic response after treatment and a relapse after few months. GS could benefit from PET for initial staging to monitor therapy and detect recurrence.
