In three female patients with cystinuria, marked proteinuria developed after two to 36 months of therapy with penicillamine (D-penicillamine). The proteinuria disappeared after cessation of treatment. We conclude that penicillamine as well as DL-penicillamine can cause serious nephrotoxicity and that all patients treated with this drug must be followed up indefinitely for evidence of this complication. The mechanism of the observed nephrotoxicity is under current investigation.
