Pyogenic liver abscess with gas visible on roentgen examination is usually rapidly progressive and often difficult to diagnose. The presence of gas within the abscess allows the radiologist to offer a definitive diagnosis. The organism in the 3 cases presented in this article was one of the klebsiellas. Differential diagnosis includes subphrenic abscess, emphysematous cholecystitis, right renal or perinephric abscess, hepatic flexure interposed between the diaphragm and liver, and partial abdominal heterotaxia.
