Percutaneous trans-septal mitral valve-in-ring implantation with elective VA-ECMO support was successfully performed in a high-risk patient, resulting in clinical improvement at 6 months.
Why the study?
Percutaneous mitral valve-in-valve implantation is an emerging option for patients with surgical bioprosthesis failure or failing mitral annuloplasty and increased surgical risk.

Demonstrates the feasibility of transcatheter mitral valve-in-ring implantation supported by elective VA-ECMO in a high-risk patient with severe LV dysfunction.
TMVinR with VA-ECMO appears feasible in severe LV dysfunction; leaves open safety, efficacy, and generalizability pending prospective data.
Percutaneous mitral valve-in-valve implantation is an emerging option in patients with surgical bioprosthesis failure or failing mitral annuloplasty and increased surgical risk. We present a case of transcatheter transvenous trans-septal mitral valve-in-ring (TMVinR) procedure, in a patient with severe left ventricular dysfunction and severe mitral regurgitation, after surgical mitral annuloplasty, managed with periprocedural mechanical circulatory support (MCS) with VA-ECMO.

Ristalli et al. (2019) conducted a case report in Severe mitral regurgitation and severely reduced left ventricular ejection fraction (n=1). Percutaneous trans-septal mitral valve-in-ring implantation with VA-ECMO was evaluated on Procedural success and clinical outcome. Percutaneous trans-septal mitral valve-in-ring implantation with elective VA-ECMO support was successfully performed in a high-risk patient, resulting in clinical improvement at 6 months.
