Thalamic fractures of the calcaneus reveal great complexity for their pretherapeutic assessment and management. Our retrospective study of 54 fractures shows result identical to others in the literature. With re-examination, factor pain is of primary importance. Indeed, it influences directly walk, resumption of professional occupation and subjective satisfaction index. Our poor results are correlated with subtalar osteoarthritis occurrence without we being able to define predictive factors. Among the prognostic factors, we retain primarily occurrence of complications. The reduction of Böhler’s angle and joint congruency are for us guarantor of good results. It must be certain in postoperative and fixation used must be able to maintain it. The study of these fractures carried out in standard X-ray is a disadvantage because complex or comminuted character of some fractures is sometimes difficult to be focused. The CT makes it possible to better refine indication of ORIF apart from types V of Duparc for which reconstruction-arthrodesis keeps all its interest. One must note good results of nonoperative treatments for type I fractures and best results of ORIF for complex fractures.
