Key result
Ablation plus pharmacologic CRT linked to restored sinus rhythm and LVEF improvement to ~60% without devices.
Why the study?
Does catheter ablation combined with pharmacologic CRT improve LVEF and symptoms in a patient with persistent AF, systolic heart failure, and LBBB without an ICD?
Case Report (n=1)
No
Does catheter ablation combined with pharmacologic CRT improve LVEF and symptoms in a patient with persistent AF, systolic heart failure, and LBBB without an ICD?
Pharmacologic CRT combined with catheter ablation can effectively restore sinus rhythm and improve LVEF in select patients with AF, LBBB, and systolic heart failure without requiring an ICD.

May support device-free resynchronization in select HF cases; hypothesis-generating and requires prospective trials before practice change.
Hideyuki Hasebe (2018) conducted a case report in Persistent atrial fibrillation, systolic heart failure, and left bundle branch block (n=1). Catheter ablation and pharmacologic cardiac resynchronization therapy (amiodarone and bisoprolol) was evaluated on Restoration of sinus rhythm, heart rate reduction, heart failure symptom reduction, and LVEF improvement. Catheter ablation followed by pharmacologic cardiac resynchronization therapy with amiodarone and bisoprolol restored sinus rhythm and improved LVEF from 30% to 60% without an implantable device.
