Key result
Open AAA repair links to higher odds of postoperative MI versus EVAR.
Why the study?
The study evaluated the incidence of postoperative myocardial infarction at 30 and 90 days following AAA repair and examined its association with surgical approach, preexisting comorbidities, and preoperative medications.
Does open surgical AAA repair compared to EVAR increase the risk of postoperative myocardial infarction in patients undergoing AAA repair?
Cohort (n=37,386)
Yes
Does open surgical AAA repair compared to EVAR increase the risk of postoperative myocardial infarction in patients undergoing AAA repair?
Open AAA repair and a history of prior myocardial infarction are significant independent predictors of postoperative myocardial infarction at 30 and 90 days.

Vidovich et al. (2025) conducted a cohort in abdominal aortic aneurysm (n=37,386). Open surgical AAA repair vs. Endovascular aneurysm repair (EVAR) was evaluated on postoperative myocardial infarction (POMI) at 30 and 90 days. Open AAA repair was associated with significantly higher odds of postoperative myocardial infarction (overall incidence 2.7% at 30 days and 3.7% at 90 days) compared to EVAR.
