To determine whether intraoperative dexmedetomidine administration reduces postoperative hepatic ischemia/reperfusion injury and improves clinical outcomes in living liver donors undergoing laparoscopic hepatectomy.
Randomized controlled trial (N=55; ChiCTR2000040629) assigning patients undergoing laparoscopic living donor hepatectomy to intravenous dexmedetomidine (1 μg/kg bolus over 15 minutes before induction, then 0.4 μg/kg/h until portal branch disconnection) or an identical volume of 0.9% saline.
Tracked the primary endpoint of peak serum aspartate aminotransferase (AST) within 72 hours postoperatively alongside secondary measures of hemodynamic stability, organ function, recovery metrics, and complications.
Peak serum AST levels across the first 72 postoperative hours did not differ significantly between groups (dexmedetomidine: 288 [194–466] IU/L vs. control: 324 [194–437] IU/L; difference: −1.2 IU/L; 95% CI, −86.9 to 88.0; P=0.973).
Intraoperative heart rate decreased significantly in the dexmedetomidine arm, but intraoperative mean arterial pressure, renal and liver function panels, and complication rates showed no significant differences.
