Propofol induction reduces first-day postoperative delirium versus etomidate in elderly orthopaedic patients.
Why the study?
Elderly patients are vulnerable to cognitive impairment due to underlying disease and nervous system changes, prompting comparison of etomidate versus propofol after orthopaedic surgery.
RCT (n=100)
Double-blind
Alternating assignment
No
Absolute Event Rate: 26.34% vs 25.28%
p-value: p=0.01
May support propofol preference in elderly orthopaedic patients; hypothesis-generating and requires larger randomized confirmation before practice change.
Elderly are more exposed to cognitive impairment due to the presence of underlying disease and some structural and functional changes in the nervous system. The aim of this study is to compare the effects of two drugs of Etomidate and Propofol on haemodynamics, delirium and dementia of elderly patients after orthopaedic surgery.

Ghavipeykar et al. (2020) conducted an RCT in Orthopaedic surgery in the elderly (n=100). Propofol vs. Etomidate (0.2 mg/Kg) was evaluated on Delirium level on the first day after surgery (NEECHAM score) (p=0.01). Propofol induction for orthopaedic surgery in elderly patients significantly reduced delirium on the first postoperative day compared to etomidate (NEECHAM score 26.34 vs 25.28, p=0.01).
