Key result

Fenofibrate in type 2 diabetes is linked to a ~13% relative increase in flow-mediated dilation.

  • MD 4.5%
  • P=0.004
  • n=27

Why the study?

Does fenofibrate improve endothelial function and inflammatory biomarkers in patients with type 2 diabetes mellitus?

Population

27 patients with type 2 diabetes mellitus and good glycemic control treated with metformin monotherapy…

Design

Cohort, open-label

Follow-up

Implication

May improve endothelial function in T2DM; hypothesis-generating and requires randomized trials before practice change.

Structured PICO

Does fenofibrate improve endothelial function and inflammatory biomarkers in patients with type 2 diabetes mellitus?

P
Population
27 adults with type 2 diabetes mellitus on stable metformin monotherapy and without prior lipid-lowering medication, followed for 8 weeks.
I
Intervention
Fenofibrate therapy for 8 weeks
O
Outcome
Endothelial function assessed by vascular studies (brachial artery diameter, flow-mediated dilation), levels of soluble E-selectin (sE-selectin), and plasma myeloperoxidase (MPO)surrogate

Main Result

Mean Difference: 4.5

Absolute Event Rate: 25% vs 22.2%

p-value: p=0.004

Short-term fenofibrate therapy improves vascular endothelial function and reduces inflammatory biomarkers in patients with type 2 diabetes.

Limitations

  • Relatively small number of patients included
  • Short duration of the fenofibrate treatment
  • Study design was not placebo-controlled

Niţă et al. (2014) studied Type 2 diabetes mellitus (n=27). Fenofibrate vs. Pre-treatment baseline was evaluated on Flow-mediated dilation (FMD) response to hyperaemia at 60 s (MD 4.5%, p=0.004). Fenofibrate treatment for 8 weeks significantly improved vascular endothelial function in patients with type 2 diabetes, increasing median flow-mediated dilation from 22.2% to 25.0% (p=0.004).
