Population

68 adults with tuberculous pericardial effusion (out of 91 evaluated patients)

Design

Cohort

Key result

Pre-pericardiocentesis right atrial pressure > 15 mmHg (OR 48) and serum IL-10 > 200 pg/ml (OR 10) were independently associated with effusive-constrictive pericarditis, which occurred in 52.9% of patients with tuberculous pericardial effusion.

Main Result

Odds Ratio: 48 (95% CI 8.7–265)

p-value: p=<0.0001

Effusive-constrictive pericarditis is highly prevalent (52.9%) in patients with tuberculous pericardial effusion and is characterized by elevated right atrial pressure and IL-10 levels.

Limitations

  • Selection bias due to enrolling only patients with moderate to large pericardial effusions amenable to safe pericardiocentesis.
  • Only 60% of cases had a proven diagnosis of tuberculosis, meaning the remaining 40% with a presumed diagnosis may have had other inflammatory causes.
  • Small sample size of 36 ECP cases limits the power of the study.

Ntsekhe et al. (2013) conducted an observational in Tuberculous pericardial effusion (n=68). Right atrial pressure > 15 mmHg vs. Right atrial pressure ≤ 15 mmHg was evaluated on Effusive-constrictive pericarditis (ECP) (OR 48, 95% CI 8.7-265, p=<0.0001).
