Rescue CABG offers a viable treatment for infants with compressive giant coronary aneurysms in Kawasaki disease.
Case Report (n=1)
Rescue CABG can be a viable treatment option for infants in the acute phase of Kawasaki disease presenting with severe coronary compression from giant aneurysms.
May support rescue CABG consideration in select acute infantile Kawasaki cases with compressive giant aneurysms; leaves open need for prospective validation.
We report a Kawasaki disease infant with a giant coronary artery aneurysm compressing the adjacent coronary artery causing significant stenosis. Our case suggests that a giant aneurysm at the left main coronary artery can cause severe left anterior descending (LAD) artery stenosis by mechanical compression, and coronary artery bypass grafting could be a useful rescue treatment option even in small infants in the acute phase.

Bae et al. (2025) conducted a case report in Kawasaki disease with giant coronary artery aneurysm (n=1). Coronary artery bypass grafting was evaluated. Coronary artery bypass grafting can be a useful rescue treatment option for infants in the acute phase of Kawasaki disease with a giant coronary artery aneurysm causing mechanical compression.
