3D TEE identifies a malpositioned left atrial pacing lead in a patient with prior TIA.
Why the study?
Inadvertent malpositioning of a cardiac pacing lead into the left heart chambers is a rare complication of transvenous pacing.
Transesophageal 3D echocardiography is a useful imaging modality for definitively delineating malpositioned permanent pacing leads.
Inadvertent malpositioning of a cardiac pacing lead into the left heart chambers is a rare complication of transvenous pacing. We report a patient with a history of a transient ischemic attack and chronic chest pain whose left atrial pacing lead location was revealed by transesophageal three-dimensional (3D) echocardiography during evaluation of an inter-atrial shunt.

Madan et al. (2021) studied this question. Transesophageal 3D echocardiography definitively identified an inadvertently malpositioned left atrial pacing lead in a patient with chronic chest pain and a history of transient ischemic attack.
