Steroid pulse therapy resolves fulminant eosinophilic myocarditis linked to the BNT162b2 COVID-19 vaccine.
Why the study?
COVID-19 vaccination is reported to be safe and effective, but the histologic features of post-COVID-19 vaccination myocarditis remain unknown.
Steroid pulse therapy can be an effective treatment for fulminant eosinophilic myocarditis occurring as a rare complication after COVID-19 vaccination.
Alerts clinicians to steroid-responsive eosinophilic myocarditis post-BNT162b2; leaves open incidence and causality in larger cohorts.
Coronavirus disease 2019 (COVID-19) vaccination is reportedly safe and effective. The histologic features of post-COVID-19 vaccination myocarditis are unknown. We present a case of a 77-year-old Japanese woman diagnosed with eosinophilic myocarditis using endomyocardial biopsy, 7 days after the second dose of BNT162b2 COVID-19 vaccine. Steroid pulse therapy was effective.

Otsuka et al. (2022) studied this question. The second dose of the BNT162b2 COVID-19 vaccine was associated with fulminant eosinophilic myocarditis in a 77-year-old woman, which responded effectively to steroid pulse therapy.
