Background: Electronic Health Record Systems (EHRS) Personal Health Record Systems (PHRS) are core components of infrastructure needed to run any health system. : As health systems undergo paradigm changes, and PHRS have to advance as well to meet the related challenges. : The paper discusses EHR types, implementations standards, starting with different requirements , systems and systems architectures, standards solutions. : Existing standards and specifications are compared changing requirements, presenting weaknesses defining the advancement of EHRS, architectures and services, embedded in advanced infrastructure systems. : Future EHR systems are components in layered architecture with open interfaces. The need verifying data models at business domains level is highlighted. Such approach is enabled by the Interoperability Reference Architecture of a systemoriented, architecture-centric, ontology-based, policy- driven approach, meeting good modeling best practices.
