Key Points

To evaluate optical coherence tomography data assessing retrograde degeneration across different anatomical lesion levels of the visual pathway.
Measured ganglion cell layer (GCL) thickness using optical coherence tomography (OCT) in 79 patients with visual pathway damage at varying anatomical levels.
Categorized visual injuries into anterior lesions affecting the optic nerves or chiasma (n=21) and post-genicular lesions (n=58; 33 temporal resections and 25 occipital lesions).
Assessed all participants using visual acuity testing, automated static perimetry, brain MRI or CT, and retinal ganglion cell complex OCT analysis.
Anterior visual pathway damage induced GCL thinning in 95.2% (20/21) of patients at 22 days or later post-injury.
Post-genicular damage resulted in GCL thinning in 43.1% (25/58) of patients, occurring in 27.3% (9/33) after temporal lobe surgery and 64.0% (16/25) with occipital lobe lesions.
The minimum duration before detectable GCL thinning was 3 months following temporal lobe surgery and 5 months following occipital lobe damage.
