Does elective spinal fusion surgery affect total and ionized magnesium concentrations?
While total hypomagnesemia occurs in 15% of patients during spinal surgery, biologically active ionized hypomagnesemia is rare (3%).
Answers are grounded in this paper and cite their sources.
Total magnesium may overestimate hypomagnesemia after spinal surgery; leaves open whether ionized monitoring affects outcomes.

Chang et al. (2007) conducted an observational in Elective spinal fusion surgery (n=30). Spinal surgery vs. Preoperative baseline was evaluated on Ionized magnesium concentration (mM/L) (p=0.0033). Spinal surgery significantly decreased total and ionized magnesium concentrations, resulting in a 15% incidence of total hypomagnesemia but only a 3% incidence of ionized hypomagnesemia.
