Swedish versions of the Care Transition Measure (CTM) was evaluated on Psychometric properties of the Swedish versions of the 15-item (CTM-15) and the 3-item (CTM-3) Care Transition Measure. A 4-item unidimensional version of the Swedish Care Transitions Measure (CTM-4) demonstrated acceptable model fit and good reliability (Cronbach's alpha 0.85), outperforming the 15-item and 3-item versions.
