SUMMARY The thyroid function and health status of 101 children with Down syndrome were assessed annually for five years. One child had congenital hypothyroidism at entry to the study. During the study period. eight more developed compensated hypothyroidism. Five of 10 children with compensated hypothyroidism still had the condition at the end of the study, i t resolved spontaneously in four and one child developed uncompensated hypothyroidism. There were no significant differences in growth and development between those with compensated hypothyroidism and those with normal thyroid function. Two children developed transient rises in thyroxine, associated with elevations in thyroid‐stimulating hormone (TSH). A large proportion of thyroid dysfunction in children with Down syndrome is transient and may be related to inappropriate secretion of TSH or thyroid insensitivity to TSH, rather than to auto‐immune thyroiditis. RÉSUMÉ Etude longitudinale de cinq ans portant sur la fonction thyroidienne d'enjants trisomiques 21 La fonction thyroidienne et I‘état de sante de 101 enfants trisomiques 21 ont été apprecies annuellement durant cinq ans. Un enfant présentait un hypothyroïdisme congenital et deux avec hypothyröidisme compense au début de I‘étude. Durant la période d‘études, huit autres enfants développèrent un hypothyroidisme compense. Sur les cinq enfants parmi les 10 avec hypothyroidisme compensé et qui presentaient toujours cet etat a la fin de I'etude, il y eu une resolution spontanée dans quatre cas et un enfant presenta une hypothyroidie decompensee. II n'y eu pas de différence significative de croissance et de dévrloppement entre les enfants avec hypothyroïdie compensée et les enfants avec fonction thyroïdienne normale. Deux enfants présentèrent une elevation provisoire du taux de thyroxine, associée a une élévation du taux d'hormone thyreostimuline (TSH). Une forte proportion de la dysfonction thyroidienne chez I'enfant trisomique 21 est transitoire et peut ětre reliée à une sécrétion inappropriée de TSH ou à une insensibilité de la thyroïde a la TSH, plut6Ct quà une thyroïdite auto‐immune. ZUSAMMENFASSUNG Eine fünfjührige Longiiudinalstudie über Schilddrusenfunktion bei Kindern mil Down Syndrom Bei 101 Kindern mit Down Syndrom wurden uber funf Jahre einmal pro Jahr die Schilddrüsenfunktion und der Gesundheitszustand untersucht. Ein Kind hatte zu Beginn der Studie eine congenitale Hypothyreose und zwei hatten kompensierte Hypothyreose. Im Verlauf der Studie entwickelten weitere acht eine kompensierte Hypothyreose. Fünf der acht Kinder mit kompensierter Hypothyreose hatten diere noch am Ende der Studie, bei vieren besserte sie sich spontan und ein Kind entwickelte eine nicht‐kompensierte Hypothyreose. Es fanden sich keine signifikanten Unterschiede in Bezug auf Wachstum und Entwicklung zwischen den Kindern mit kompensierter Hypothyreose und denen mit normaler Schilddriisenfunktion. Zwei Kinder hatten vorübergehende Erhöhungen von Thyroxin, verbunden mit Erhöhungen des Thyreoidea‐stimulierenden Hormons (TSH). Ein groner Teil der Schilddrüsenfunktionsstörungen bei Kindern mit Down Syndrom ist transitorisch und wahrscheinlich eher auf eine unzureichende Sekretion von TSH oder eine verminderte Ansprechbarkeit der Schilddrüse auf TSH zuruckzfuhren als auf eine Autoimmun‐Thyreciditis. RESUMEN Estudio longitudinal de cinco anos de la futnción tiroidea en nifios con sindrome de Down Durante cinco anos se evaluo la funcion tiroidea y el estatus de salud de 101 ninos con sindrome de Down. Al iniciar el estudio un nino tenia un hipotiroidismo conginito y dos tenán hipotiroidismo compensado. Durante la duracion del estudio otros ocho ninos desarrollaron un hipotiroidismo compensado. Cinco de los 10 niilos con hipotiroidismo compensado todavía lo sufrian al final del estudio, en cuatro se resolvio espontáneamente y en un caso sc desarrollo un hipotiroidisnio incompensado. No habian diferencias significativas en el crecimiento y desarrollo entre los casos con hipotiroidismo compensado y los que tenian una función tiroidea normal. Dos ninos sufrieron aumentos transitorios de tiroxina, asociados a elevacion de la hormona tiroestimulante (TSH). Una gran proporción de disfuncción tiroidea en ninos con sindrome de Down es transitorfa y puede estar en relacion con una inapropiada secreción de TSH o con una insensibitidad tiroidea a la TSH. más que a una tiroiditis por autoinmunización.
