Prophylactic pacemaker prevents further AV conduction deterioration in an infant with progressive heart block.
Why the study?
Progression of atrioventricular conduction abnormalities in infants born to anti-Ro positive mothers with antenatal myocardial dysfunction is not well characterized.
Case Report (n=1)
This case highlights the potential for progressive atrioventricular conduction disease in infants born to mothers with anti-Ro and anti-La antibodies, which may necessitate prophylactic pacemaker implantation.
May warrant consideration in select anti-Ro/La cases with progressive block; leaves open need for prospective data before broader adoption.
A child of a mother with maternal anti-Ro and anti-La antibodies presented antenatally with abnormal myocardial function, and was found to have a first degree heart block at birth. The extent of the abnormality in the conduction system progressed, with appearance of left bundle branch block in addition to further prolongation of PR interval. A pacemaker was implanted prophylactically, but patient has remained well, with no further deterioration in her atrioventricular conduction.

Uzun et al. (1999) conducted a case report in Progressive disease of the atrioventricular conduction axis (n=1). Prophylactic pacemaker implantation was evaluated on Atrioventricular conduction deterioration. Prophylactic pacemaker implantation in an infant with progressive heart block born to an anti-Ro/La positive mother prevented further deterioration in atrioventricular conduction.
