RIPC reduces plasma MHRT expression ~15-fold vs sham in patients undergoing CABG.
Why the study?
The effect of remote ischemic preconditioning on hemodynamic parameters and lncRNA MHRT expression during isolated coronary artery bypass graft surgery was unknown.
Does remote ischemic preconditioning alter MHRT expression and hemodynamic parameters in patients with coronary artery disease undergoing coronary artery bypass graft surgery?
Does remote ischemic preconditioning alter MHRT expression and hemodynamic parameters in patients with coronary artery disease undergoing coronary artery bypass graft surgery?
Remote ischemic preconditioning during CABG significantly decreases MHRT expression in myocardium and plasma, suggesting its potential as a cardiac biomarker for ischemic protection.
RIPC-associated MHRT reduction may indicate ischemic protection in CABG; leaves open biomarker validity and clinical outcome impact pending larger trials.
Aim.This study aimed to access the effect of remote ischemic preconditioning (RIPC) on hemodynamic parameters and expression of lncRNA MHRT in patients with coronary artery disease during isolated coronary artery bypass graft surgery.Methods.The hemodynamic parameters of 29 patients with sham and RIPC procedure were analyzed.RIPC was modulated by three cycles of blocking blood flow in the arm for 5 minutes followed by restoring it for another 5 minutes.Expression of MHRT was measured in myocardial tissue and plasma by real-time polymerase chain reaction.Results.Hemodynamic parameters of patients in the RIPC-group had higher (p<0.05)values of cardiac index, systolic blood pressure, diastolic blood pressure, stroke volume index compared to the sham group.Systemic vascular resistance index decreased in RIPC-group.The expression level of MHRT in the myocardium was 2.5 times lower than in the sham group (p<0.05).MHRT expression in plasma fall[s] significantly, by more [than]15 times, (p<0.0001) in the RIPC-group.Conclusions.A decrease in MHRT expression can be a consequence of the protective effect of RIPC and can be used as a cardiac biomarker.

Khetsuriani et al. (2021) studied Coronary artery disease (n=29). Remote ischemic preconditioning (RIPC) vs. Sham procedure was evaluated on MHRT expression in plasma (>15 times lower, p=<0.0001). Remote ischemic preconditioning significantly decreased MHRT expression in plasma by more than 15-fold (p<0.0001) and in myocardium by 2.5-fold (p<0.05) compared to sham in patients undergoing CABG.
