Perioperative sildenafil reduces ICU stay in pediatric CHD with PH but yields no mortality benefit.
Why the study?
The potential benefits of perioperative sildenafil therapy in pediatric patients with congenital heart disease on ICU stay and mortality were unclear.
Does perioperative sildenafil therapy reduce the length of ICU stay in pediatric congenital cardiac disease patients?
Meta-Analysis (n=238)
Does perioperative sildenafil therapy reduce the length of ICU stay in pediatric congenital cardiac disease patients?
Perioperative sildenafil may reduce ICU length of stay in pediatric CHD patients, though it does not appear to impact pre-discharge mortality or total hospitalization length.
Shortens ICU stay without mortality benefit; extends perioperative sildenafil evidence in pediatric CHD-PH beyond prior endpoints.
= 84%, P < 0.00001), which had a high heterogeneity. The findings were robust after the sensitivity analyses. The perioperative treatment of sildenafil for CHD pediatric patients is a potential method to reduce the length of ICU stay. We observed no differences with the use of it in the mortality before discharge and the length of hospitalization.

Jiang et al. (2018) conducted a meta-analysis in Pulmonary hypertension secondary to congenital heart disease (n=238). Sildenafil vs. Placebo, no treatment, or milrinone was evaluated on Mortality before discharge (RR 0.35, 95% CI 0.06-2.10, p=0.25). Perioperative sildenafil in pediatric patients with congenital heart disease and pulmonary hypertension reduced ICU stay (MD -18.18) but did not significantly affect mortality (RR 0.35).
