Prothrombin complex concentrate successfully manages life-threatening pericardial bleeding complicating AF ablation during edoxaban therapy.
Why the study?
Management of bleeding complications during AF ablation in patients on edoxaban remains challenging due to lack of licensed reversal agents for anti-factor Xa DOACs.
Does prothrombin complex concentrate successfully manage life-threatening pericardial bleeding in patients on edoxaban undergoing atrial fibrillation ablation?

Does prothrombin complex concentrate successfully manage life-threatening pericardial bleeding in patients on edoxaban undergoing atrial fibrillation ablation?
Prothrombin complex concentrate may be an effective strategy for managing life-threatening bleeding complications in patients on edoxaban who lack a specific licensed reversal agent.

May inform DOAC-related pericardial bleed management in AF ablation; hypothesis-generating and requires prospective validation before practice change.
The risk of pericardial bleeding complicating atrial fibrillation (AF) ablation is estimated to be around 0.5%–1%.1 The consequences may be exacerbated by the need for anticoagulation for prophylaxis of thromboembolism. Once-daily administered direct oral anticoagulants (DOACs) such as edoxaban can be safely continued uninterrupted at the time of AF ablation.2 However, management of bleeding complications in the context of DOAC therapy remains a challenge, especially as anti-factor Xa DOACs do not currently have licensed specific reversal agents.

Choudhury et al. (2019) conducted a case report in Atrial fibrillation. Prothrombin complex concentrate was evaluated. Prothrombin complex concentrate successfully managed a life-threatening pericardial bleed complicating atrial fibrillation ablation in a patient on edoxaban therapy.
