Muscle is usually recommended for packing the sella after trans-sphenoidal hypophysectomy. However, muscle atrophies when transplanted, and a recent case in which visual loss recurred twice due to downward migration of the chiasm after chiasmapexy by packing the sella with muscle suggests strongly that fat should be used instead of muscle to pack the sella after trans-sphenoidal operations.
