An outbreak of acute diarrhoeal disease followed a mass gathering in Shivamogga, Karnataka, in January 2023. We investigated the source. Of the 1,337 event attendees, 720 were interviewed and 268 met the case definition (attack rate 37%). The epidemic curve indicated a point-source outbreak with median incubation of 34 h. Shigella spp. were isolated from two stool samples. Water testing at the catering site showed faecal contamination and pipeline damage. Consumption of uncooked coconut chutney was strongly associated with the outbreak [Odds ratio (OR) 7.6, 95% confidence interval (CI) 3.5–16.4], suggesting a foodborne outbreak linked to contaminated water used in food preparation.
