Elevated insulin triggers a ~2-fold increase in cardiac MSC collagen expression via abnormal fibroblast differentiation.
Why the study?
Age-related cardiac fibrosis involves increased collagen despite reduced TGF-β responsiveness, potentially linked to elevated circulating insulin and defective MSC differentiation.
Effect estimate: 2-fold increase
Elevated insulin levels in aging promote aberrant cardiac MSC differentiation into fibroblasts, driving a TGF-β-independent mechanism of cardiac fibrosis.

Hyperinsulinemia may promote cardiac fibrosis via MSC differentiation; this in vitro finding leaves open in vivo relevance and clinical implications.
Cieslik et al. (2013) studied Cardiac fibrosis in aging. Elevated circulating insulin levels vs. Young cardiac MSCs / untreated aged fibroblast cultures was evaluated on Collagen expression (2-fold increase). Elevated insulin levels (1 nM) caused abnormal fibroblast differentiation of young cardiac MSCs, leading to a 2-fold increase in collagen expression, suggesting an insulin-dependent mechanism of fibrosis.
