In healthy Japanese individuals, elevated NT-proBNP appears to be driven by increased cardiac preload and afterload rather than impaired systolic function.
In apparently healthy Japanese subjects, both increased cardiac preload and increased cardiac afterload, but not impaired cardiac systolic function, may be associated with elevated serum NT-proBNP.

Iwasaki et al. (2019) conducted an observational in Healthy subjects (n=2,844). Increased cardiac preload (FIB-4 score) and afterload (PP2) vs. Unexposed/reference baseline was evaluated on Correlation between change in cardiac preload (FIB-4 score) and afterload (PP2) with change in serum NT-proBNP (r=0.09 for FIB-4, r=0.05 for PP2, p=<0.01). In apparently healthy Japanese subjects, increased cardiac preload and afterload, but not impaired cardiac systolic function, were significantly associated with elevated serum NT-proBNP.
