A BSTRACT Pilonidal sinus disease is a chronic, debilitating illness affecting young males. Although its existence dates back over two centuries, the exact pathogenic mechanism remains unresolved. Numerous theories have been postulated, but a definitive etiology remains hypothetical. We report a case of pilonidal sinus and review the pathogenic mechanisms, shedding some light on the likely pathway of braided hair tuft formation and migration by integrating all the available evidence.
