Meta-ethnography reveals multidimensional emotional, structural, and systemic challenges in caregivers of youths with chronic illness, highlighting the need for tailored transition support.
Background The transition from paediatric to adult healthcare for adolescents with chronic conditions is a pivotal yet challenging phase for caregivers, given their crucial role in preparing their child for this transition. This process involves complex adjustments in roles, responsibilities and navigation of healthcare systems. To address these challenges, we synthesized the qualitative experiences of caregivers to better understand their perspectives and readiness during this transition process, highlighting the emotional, structural and systemic factors involved. Methods A systematic review and meta‐synthesis were conducted using Noblit and Hare's meta‐ethnographic approach. Systematic searches were performed across PubMed, Scopus, CINAHL and PsycINFO. Data analyses involved extracting first‐ and second‐order constructs and synthesizing them into third‐order constructs, generating a novel conceptual framework that reflects experiences and needs of caregivers. Results The database search yielded 9306 studies. After applying inclusion and exclusion criteria, 23 qualitative studies were included. Analyses identified five key themes: (1) Caregiver Empowerment and Advocacy, where caregivers emphasized advocating for their needs within healthcare, employment and education systems; (2) Building Networks and Collaboration, highlighting the importance of collaborative networks for seamless transitions; (3) Emotional and Psychological Preparedness, indicating the need for emotional and psychological resources among caregivers; (4) Structural and Systemic Support, emphasizing the role of structured programs, communication technologies and cross‐sectoral support systems; and (5) Tailored Transition Plans and Professional Integration, where caregivers play a critical role in advocating for individualized plans that build skills and confidence in adolescents as they navigate healthcare and employment opportunities. Conclusions This meta‐ethnography emphasizes the multifaceted roles of caregivers during their child's healthcare transition. The findings highlight the need for structured support systems that empower caregivers, provide emotional resources and foster collaborative networks. Tailored transition plans addressing both the needs of adolescents and caregivers are essential. Further research should explore interventions that enhance caregiver readiness—the preparedness, confidence and knowledge caregivers need to support youth during transitions as well as the efficacy of transition support programs.
