鈍的胸部外傷による心損傷は死亡率が高く，アプローチとして胸骨正中切開だけでなくClamshell thoracotomyも選択肢の1つとなりうる．今回われわれは鈍的胸部外傷による左心耳損傷に対してClamshell thoracotomyにて止血を得た症例を経験したので報告する．症例は49歳女性．夫との口論後に6階の自宅ベランダから飛び降りて腹臥位で落下した．ショックで搬送され，FASTにて心嚢液を認めた．心タンポナーデにより心停止となり，左前方側方開胸を実施し心膜を開窓した．心嚢内の血腫を除去したところで自己心拍再開したが多量の血液噴出を認め，視野確保目的にClamshell thoracotomyに移行した．左心耳に約8 mmの損傷を認め，ECMOを確立した後に縫合止血した．術後20日目にICUを退室し，術後77日目に独歩自宅退院した．鈍的心損傷の原因として交通外傷が多いが，本症例は墜落外傷による左心耳損傷でありClamshell thoracotomyにて救命できた非常に稀な症例である．またClamshell thoracotomyは臨床現場において開胸手段の1つとなるため，十分に認知しておくことが必要である．
