Pericardiectomy and antifungal therapy successfully treats cryptococcal constrictive pericarditis in an immunosuppressed patient.
Why the study?
Cryptococcal pericarditis is rare and its diagnosis and management in immunosuppressed patients with pericardial disease require clarification.
Case Report (n=1)
No
Cryptococcal pericarditis should be considered in immunosuppressed patients with pericardial effusion or thickening, and a combined surgical and antifungal approach is essential.
May support combined pericardiectomy and antifungal therapy in rare cryptococcal constrictive pericarditis; hypothesis-generating and requires prospective validation.
Introduction: involvement is exceptional outside the context of HIV but may occur in immunosuppressed patients. Case description: infection. She received antifungal therapy (liposomal amphotericin B plus flucytosine, followed by fluconazole) with a favourable 12-month outcome. Discussion: This case illustrates the importance of considering fungal aetiologies in immunosuppressed patients with pericardial disease. Pericardiectomy was both diagnostic and therapeutic. Antifungal therapy according to international guidelines ensured clinical success. Conclusion: Cryptococcal pericarditis, though rare, should be considered in immunosuppressed patients with pericardial effusion or thickening. A combined approach with surgery and antifungals is essential. LEARNING POINTS: Cryptococcal pericarditis is rare but should be suspected in immunosuppressed patients with pericardial effusion or thickening.Pericardiectomy is crucial as both a diagnostic and therapeutic intervention in constrictive pericarditis.Sequential antifungal treatment, in accordance with international guidelines, leads to favourable outcomes.

Silva et al. (2026) conducted a case report in Constrictive pericarditis due to Cryptococcus neoformans (n=1). Pericardiectomy and antifungal therapy was evaluated on Clinical outcome. A combined approach of pericardiectomy and sequential antifungal therapy successfully treated cryptococcal constrictive pericarditis in an elderly immunosuppressed patient, yielding a favorable 12-month outcome.
