Why the study?
The role and outcomes of femoropopliteal bypass surgery for lower limb revascularization require synthesis according to various national and international guidelines.
What is the role and optimal graft choice for femoropopliteal bypass surgery in lower limb revascularization?
What is the role and optimal graft choice for femoropopliteal bypass surgery in lower limb revascularization?
Biological prostheses for femoropopliteal bypass are associated with poor long-term outcomes due to increased thrombosis and restenosis compared to other graft types.
Supports caution with biological prostheses in femoropopliteal bypass; leaves open optimal graft selection for randomized trials.
This review is devoted to the role of femoropopliteal bypass (FPB) surgery for lower limb revascularization. The authors discuss the indications for surgery and choice of reconstruction method according to the Russian guidelines (national guidelines for the diagnosis and treatment of peripheral artery disease (Moscow, 2019), clinical guidelines “Diseases of the lower limb arteries” (Ministry of Health of the Russian Federation, 2016)), guidelines of the European Society of Cardiology and the European Society of Vascular Surgeons (2017), Transatlantic Consensus (TASC II). A special attention is paid to the results of FPB with autologous veins, biological and polytetrafluoroethylene prostheses. New methods and techniques of surgery improving patency of the graft are presented. Implantation of biological prosthesis is associated with higher incidence of thrombosis, restenosis and aneurysmal transformations. Their use is characterized by non-optimal long-term results of revascularization.
