Ticagrelor rescue therapy reduces PRU by ~96% and prevents recurrent subacute stent thrombosis.
Why the study?
Stent thrombosis remains a rare but serious complication after drug-eluting stent implantation and can be associated with clopidogrel resistance.
Case Report (n=1)
No
Absolute Event Rate: 10% vs 275%
Ticagrelor can be used as an effective rescue therapy for subacute stent thrombosis in patients with clopidogrel resistance.
Ticagrelor may succeed as rescue in clopidogrel-resistant subacute ST; leaves open need for randomized confirmation.
Stent thrombosis (ST) is a rare but catastrophic complication of a drug-eluting stent.Although dual antiplatelet therapy with aspirin and clopidogrel significantly reduces the occurrence of ST, it continues to occur and is occasionally associated with clopidogrel resistance. Here, we describe a 71-yr-old man with subacute stent thrombosis and clopidogrel resistance following drug-eluting stent implantation who underwent successful ticagrelor rescue therapy.(Korean J Med 2014;87:598-602)

Ko et al. (2014) conducted a case report in Subacute stent thrombosis (n=1). Ticagrelor vs. Clopidogrel (prior therapy) was evaluated on Platelet reactivity (PRU). Ticagrelor rescue therapy successfully reduced platelet reactivity from 275 PRU to 10 PRU and prevented symptom recurrence over 6 months in a patient with subacute stent thrombosis.
