Cascade screening shows no significant reduction in cardiovascular events vs opportunistic screening after matching.
Why the study?
Whether family cascade screening for heterozygous familial hypercholesterolaemia is associated with a reduction in cardiovascular events compared with opportunistic screening remained to be determined.
Does cascade screening reduce cardiovascular events compared to opportunistic screening in patients with heterozygous familial hypercholesterolaemia?

Does cascade screening reduce cardiovascular events compared to opportunistic screening in patients with heterozygous familial hypercholesterolaemia?
Absolute Event Rate: 8.3% vs 26.5%
p-value: p=<0.001
Cascade screening for familial hypercholesterolaemia is associated with significantly fewer cardiovascular events, likely driven by earlier identification and statin initiation.

Giorgi et al. (2025) conducted a cohort in Heterozygous familial hypercholesterolaemia (HeFH) (n=3,232). Cascade screening vs. Opportunistic screening (index cases) was evaluated on Cardiovascular events prior to the first visit (p=<0.001). Cascade screening was associated with fewer cardiovascular events than opportunistic screening (8.3% vs 26.5%, P<0.001), though this association was no longer significant after age- and sex-matching.
