Key result
Prolonged chronic ischemia improves myocardial function ~1.5-fold and enhances flow reserve at 7 weeks.
Why the study?
The time dependence and spatial progression of cardiac function and angiogenesis signaling during chronic myocardial ischemia were not well characterized.
Comparison
7 weeks of ischemia vs 4 weeks of ischemia
Design
Preclinical study in porcine model
Follow-up
4 or 7 weeks

Should not change clinical practice; hypothesis-generating for spontaneous recovery in chronic ischemia and leaves open human translation.
Effect estimate: 1.5-fold improvement
In a porcine model of chronic myocardial ischemia, myocardial perfusion, function, and angiogenic signaling spontaneously improve over time (7 weeks vs 4 weeks) in both ischemic and adjacent normal territories.
Robich et al. (2010) studied Chronic myocardial ischemia (n=14). 7 weeks of chronic myocardial ischemia vs. 4 weeks of chronic myocardial ischemia was evaluated on Myocardial function (+dP/dt) (1.5-fold improvement). In a porcine model of chronic myocardial ischemia, myocardial function (+dP/dt) improved 1.5-fold and flow reserve significantly increased at 7 weeks compared with 4 weeks.
