Pericapsular nerve group (PENG) block significantly reduced pain scores during positioning for spinal anesthesia compared to fascia iliaca block in patients undergoing hip fracture surgery.
Why the study?
Does Pericapsular Nerve Group block improve analgesia compared to Fascia Iliaca Block in elderly patients undergoing hip surgery under spinal anesthesia?

Does Pericapsular Nerve Group block improve analgesia compared to Fascia Iliaca Block in elderly patients undergoing hip surgery under spinal anesthesia?
Absolute Event Rate: 0.7% vs 3.1%
p-value: p=<0.001
The abstract only reports baseline VAS scores, which were comparable between the PENG and FIB groups prior to nerve block for hip surgery.
May reduce positioning pain for spinal anesthesia; hypothesis-generating and requires randomized confirmation before practice change.
Introduction: For evaluating per capsular nerve group (PENG) block’s analgesic effect on elderly patient’s suffering from femoral neck fracture undergoing hip arthroplasty to provide a basis for optimizing perioperative analgesia in hip arthroplasty. Material and Method: This is a Prospective Observational study conducted in patients scheduled to undergo surgery for hip fracture under spinal anesthesia in Department of Anaesthesiology, Sapthagiri Institute of Medical Sciences and Research centre, Bengaluru, Karnataka. As per previous studies a total of 40 patients were included in the study over a period of 6 months. A prospective study was conducted after obtaining approval by the institutional ethics committee and informed written consent, patient scheduled for hip fracture surgery under spinal anesthesia based on inclusion criteria was randomly divided into two groups as group FIB and group PENG. Results: Total 40 patients were included in the current study. The demographic data of both the groups are presented. There was no statistically significant difference in both groups with respect to demographic characteristics. VAS score for pain before nerve block between Group P (8.4 ± 0.58) and Group F (8.1 ± 0.61) was comparable (p = 0.9983).

Krishnamurty et al. (2022) conducted an RCT in Hip fracture (n=80). Pericapsular nerve group (PENG) block vs. Fascia iliaca block (FIB) was evaluated on VAS score 30 minutes after nerve block during positioning (p=<0.001). Pericapsular nerve group (PENG) block significantly reduced pain scores during positioning for spinal anesthesia compared to fascia iliaca block in patients undergoing hip fracture surgery.
