Harmony TPV achieves successful implantation without complications in a patient with a prior mechanical aortic valve.
Why the study?
The risks of technical challenges or complications of TPVI in patients with prior aortic valve replacement using a mechanical valve remain unknown.
Does transcatheter pulmonary valve implantation prevent complications and technical difficulties in a patient with prior mechanical aortic valve replacement?

Does transcatheter pulmonary valve implantation prevent complications and technical difficulties in a patient with prior mechanical aortic valve replacement?
This case demonstrates the feasibility and safety of Harmony TPV implantation in a patient with a prior mechanical aortic valve, without coronary compression or device fracture.
Supports Harmony TPV feasibility with prior mechanical aortic valve; leaves open broader safety and applicability pending prospective data.
The Harmony™ transcatheter pulmonary valve (TPV) is specifically designed to treat severe pulmonary regurgitation in a native or surgically repaired right ventricular outflow tract. To date, no reports have described TPV implantation (TPVI) in patients with prior aortic valve replacement (AVR) using a mechanical valve. Therefore, the risks of technical challenges or complications, particularly coronary compression or device fracture associated with prior AVR, remain unknown. This is the first report of successful TPVI using a Harmony™ TPV in a 52-year-old Asian man diagnosed with tetralogy of Fallot (TOF) who had previously undergone AVR following TOF repair with no associated technical difficulties or complications.

Shima et al. (2025) conducted a case report in Severe pulmonary regurgitation with prior mechanical aortic valve replacement (n=1). Harmony Transcatheter Pulmonary Valve implantation was evaluated on Procedural success and absence of complications. Transcatheter pulmonary valve implantation using a Harmony TPV was successfully performed without technical difficulties or complications in a patient with a prior mechanical aortic valve.
