Upper-eyelid heaviness and reduced visible eyelid space are common concerns in aesthetic practice and may result from a combination of skin laxity, brow position, reduced tissue firmness and muscular activity. While significant dermatochalasis may require surgical blepharoplasty, micro-focused ultrasound with visualisation (MFU-V) offers a further option for appropriately selected patients seeking gradual, non-surgical improvement. This article explores the role of MFU-V in the upper face, including patient assessment, treatment selection, mechanisms of action, current evidence, safety considerations and expectation management. A clinical case demonstrates early improvement in brow support and visible upper-eyelid space following a single MFU-V treatment. The article emphasises the importance of assessing the underlying anatomical contributors to ageing rather than defaulting to injectable treatments, and highlights MFU-V as a complementary option within an individualised, anatomy-led approach to upper-face rejuvenation.
