CT perfusion-guided emergency ascending aorta replacement yields intact neurological survival in acute type A dissection.
Why the study?
Acute Stanford type A aortic dissection complicated by cerebral malperfusion presents a critical clinical scenario requiring urgent intervention, but detailed case data remain limited.

Preoperative CT perfusion may be useful in predicting neurological improvement after surgical repair of acute type A aortic dissection complicated by cerebral malperfusion.
Should not change practice based on one case; hypothesis-generating for CT perfusion predicting recovery in type A dissection with malperfusion.

Sakaguchi et al. (2023) conducted a case report in Stanford type A acute aortic dissection with cerebral malperfusion (n=1). Emergency ascending aorta replacement guided by CT perfusion was evaluated on Neurological outcome and survival. Emergency ascending aorta replacement guided by preoperative CT perfusion resulted in survival without neurological complications in a patient with acute type A aortic dissection and cerebral malperfusion.
