Dobutamine stress echo reveals silent extensive ST elevation unmasking severe multivessel CAD.
Why the study?
ST-segment elevation during dobutamine stress echocardiography is an uncommon finding associated with extensive coronary artery disease and its diagnostic and prognostic significance requires clarification.
Case Report (n=1)
ST-segment elevation during dobutamine stress echocardiography is an important marker that can indicate severe, multivessel coronary artery disease.
Asymptomatic ST elevation during dobutamine stress echo may signal extensive CAD in preoperative assessment; this case leaves open its prognostic value and management impact.
Pharmacological stress echocardiography is an established modality of cardiac assessment before noncardiac surgery. Electrocardiographic changes during stress echocardiography aid in the risk stratification of patients. ST-segment elevation is an uncommon finding during dobutamine stress echocardiography and is associated with extensive coronary artery disease. We report about a positive dobutamine stress echocardiography testing of an elderly patient, who developed asymptomatic, extensive ST-segment elevation during the procedure. Coronary angiography revealed severe, multivessel coronary artery disease, which was treated with percutaneous coronary intervention.

Dey et al. (2024) conducted a case report in Coronary artery disease (n=1). Dobutamine stress echocardiography was evaluated on Diagnosis of severe, multivessel coronary artery disease. Dobutamine stress echocardiography in an elderly patient revealed asymptomatic, extensive ST-segment elevation, leading to the diagnosis and treatment of severe, multivessel coronary artery disease.
