Novel endovascular salvage successfully resolves displaced aortic cuff during TAVR in prior EVAR.
Why the study?
Delayed displacement of aortic endograft components during TAVR is a rare complication with limited management strategies described.
Case Report (n=1)
This case highlights a rare complication of delayed device displacement during TAVR in patients with prior aortic stent grafts and demonstrates a novel endovascular salvage strategy.
Clinicians should consider endograft displacement risk during post-EVAR TAVR; this case leaves open generalizability of endovascular salvage.
We report the case of a 73-year-old man with a history of prior endovascular abdominal aortic aneurysm repair (EVAR) complicated by type Ia endoleak requiring two proximal aortic cuffs. Ten years later, he underwent transfemoral transcatheter aortic valve replacement (TAVR) for severe aortic stenosis. During delivery of the TAVR device, one of the aortic cuffs was displaced into the distal thoracic aorta by the delivery of the TAVR device. This case illustrates a rare complication of delayed device displacement during TAVR and demonstrates a novel strategy for endovascular salvage. It highlights opportunities during TAVR planning in patients with prior aortic stent grafts, and the value of close multidisciplinary collaboration to manage unexpected device interactions.

Punit et al. (2026) conducted a case report in Severe aortic stenosis and prior EVAR (n=1). Transfemoral transcatheter aortic valve replacement (TAVR) was evaluated on Device displacement and endovascular salvage. During transfemoral TAVR in a patient with prior EVAR, an aortic cuff was displaced into the distal thoracic aorta, requiring a novel endovascular salvage strategy.
