A modified surgical technique successfully treated a 79-year-old woman with extensive mitral valve endocarditis and posterior mitral annular calcification by reconstructing a new annulus without decalcifying the calcification.
Why the study?
The concomitant presence of posterior mitral annular calcification and infectious mitral valve lesions poses a technical challenge with considerable perioperative risk using previously proposed mitral valve surgery techniques.
Case Report (n=1)
A modified surgical technique avoiding decalcification of extensive mitral annular calcification by using bovine pericardium to reconstruct a new annulus was successfully used to treat a patient with complicated mitral infective endocarditis.
The concomitant presence of posterior mitral annular calcification and infectious mitral valve lesions poses a technical challenge with considerable perioperative risk when using previously proposed techniques for mitral valve surgery. Herein, we report a case of the use of a modified surgical technique to successfully treat a patient with mitral infective endocarditis complicated by a subendocardial abscess and extensive posterior mitral annular calcification.

Kim et al. (2016) conducted a case report in Mitral infective endocarditis with extensive posterior mitral annular calcification (n=1). Modified surgical technique (reconstructing a new annulus with plication between the left atrial wall and posterior mitral leaflet, reinforcing with bovine pericardium without decalcifying the MAC) was evaluated on Successful treatment and valve function. A modified surgical technique successfully treated a 79-year-old woman with extensive mitral valve endocarditis and posterior mitral annular calcification by reconstructing a new annulus without decalcifying the calcification.
