MVR with TV repair and maze immediately normalizes post-endocarditis thrombocytopenia in a single case.
Why the study?
The mechanism and treatment of new-onset thrombocytopenia after treated mitral valve endocarditis remain unclear.
Does valve surgery improve platelet recovery in patients with thrombocytopenia after treated infective endocarditis?

Does valve surgery improve platelet recovery in patients with thrombocytopenia after treated infective endocarditis?
Valve surgery may contribute to platelet recovery in selected patients with thrombocytopenia developing after treated infective endocarditis.
May normalize platelets refractory to steroids in mitral stenosis; leaves open causality in this single case.
/μL during routine outpatient follow-up after discharge. Follow-up echocardiography showed resolution of the vegetation, and inflammatory markers had normalized; however, corticosteroid treatment did not improve the platelet count. Elective surgery was performed for severe mitral stenosis with moderate tricuspid regurgitation and atrial fibrillation. After mitral valve replacement with concomitant tricuspid valve repair and a maze procedure, the platelet count normalized immediately and remained within the normal range during approximately 3 months of postoperative follow-up, without further thrombocytopenia-directed therapy. This case suggests a possible valve- and atrium-related mechanism for thrombocytopenia developing after treated IE and indicates that valve surgery may contribute to platelet recovery in selected patients.

Jung et al. (2026) conducted a case report in Mitral valve endocarditis with new-onset thrombocytopenia (n=1). Mitral valve replacement with concomitant tricuspid valve repair and a maze procedure was evaluated on Platelet count normalization. Mitral valve replacement with concomitant tricuspid valve repair and a maze procedure immediately normalized platelet counts in a patient with thrombocytopenia after treated mitral valve endocarditis.
