Key result
Emergency surgical thrombectomy and endarterectomy successfully resolve massive PE with mobile RV thrombi.
Why the study?
Right heart thrombus is rare and high-risk in paediatrics, and optimal management strategies including the role of surgery remain critical to define.
Case Report (n=1)
Multimodal imaging and timely surgical intervention can be lifesaving for high-risk right heart thrombi in pediatric patients failing anticoagulation.

May support emergency surgery for refractory pediatric right heart thrombus with PE; leaves open standardized protocols pending larger studies.
Right heart thrombus is rare and high-risk in paediatrics. A 14-year-old boy presented with dizziness and leg numbness. Imaging revealed mobile right ventricular thrombi and massive pulmonary embolism. Despite therapeutic anticoagulation, respiratory failure ensued. Emergency surgical thrombectomy and pulmonary endarterectomy were lifesaving. This case highlights that for high-risk right heart thrombi, multimodal imaging is key, and timely surgery is critical.
