Background Cancer is a leading cause of mortality and reduced life expectancy worldwide. Nurses play a critical role in ensuring patient safety and improving the quality of healthcare services. Therefore, the present study aimed to translate and examine the cross-cultural psychometric properties of the Persian version of the Quality of Oncology Nursing Care Scale (QONCS). Methods This descriptive cross-sectional study was conducted in five phases: (1) translation and cultural adaptation of the instrument for the Iranian context, (2) assessment of face and content validity, (3) pilot testing, (4) field testing, and (5) psychometric evaluation. Data were collected between October 2024 and March 2025. Results Following forward and backward translation and cross-cultural adaptation, the questionnaire was adapted into a 32-item version. Exploratory factor analysis initially identified a four-factor structure. However, this model was not supported by confirmatory factor analysis. After removing items Q20, Q31, and Q32, a 29-item, the three-factor model demonstrated relatively fit than the alternative models; however, the overall fit indices did not reach commonly recommended thresholds for good model fit (CFI =0.870, TLI = 0.848, and RMSEA = 0.100). Therefore, these findings should be interpreted as preliminary evidence for the proposed three-factor structure rather than as definitive confirmation of the factor structure. The final scale demonstrated high internal consistency (Cronbach's α = 0.97) . Conclusion The findings provide preliminary evidence regarding the reliability and construct validity of the Persian version of the QONCS. Further studies using larger and independent samples are needed to confirm its factor structure and establish its measurement properties.
