Pacemaker fails to prevent fatal pulmonary complications in complex neonatal heart block with TOF.
Why the study?
The coexistence of tetralogy of Fallot and congenital complete heart block linked to maternal anti-Ro antibodies in a fetus with Down syndrome presents unique diagnostic and management challenges.
Case Report (n=1)
This case highlights the diagnostic and management challenges of a rare coexistence of tetralogy of Fallot, complete heart block linked to maternal autoantibodies, and Down syndrome.
Alerts clinicians to complex management in neonates with anti-Ro heart block, tetralogy of Fallot, and trisomy 21; leaves open the interaction between autoantibodies and chromosomal anomalies.
We report a rare fetal presentation of tetralogy of Fallot and complete atrioventricular block linked to maternal anti-Ro antibodies. However, trisomy 21 was confirmed postnatally. Despite pacemaker implantation, the neonate died on day 55 due to pulmonary complications. This complex case highlights diagnostic and management challenges involving tetralogy of Fallot, Down syndrome, and complete heart block associated with maternal autoantibodies.

Chankhunaphas et al. (2026) conducted a case report in Tetralogy of Fallot, complete atrioventricular block, and Down syndrome (n=1). Pacemaker implantation was evaluated on Clinical outcome. A neonate with tetralogy of Fallot, complete heart block linked to maternal autoantibodies, and Down syndrome died on day 55 from pulmonary complications despite pacemaker implantation.
