A 31-year-old G5P5 woman with a BMI of 40 kg/m2, severe dysmenorrhea, and a history of one prior cesarean delivery presented with chronic cyclic abdominal pain localized to the left lateral margin of her Pfannenstiel incision scar. Magnetic resonance imaging showed two enhancing T2 hypointense nodules with T1 susceptibility artifact along the lower rectus musculature measuring 3 cm and 2.5 cm at the midline and left lateral aspect of the surgical scar, respectively (Fig 1). The masses were 33mm and 46mm deep in the subcutaneous tissue and were not appreciated on physical examination, but these areas were significantly tender.
