Valve-sparing root replacement with bicuspidization reduces severe aortic regurgitation to mild and reverses LV remodeling.
Why the study?
Unicuspid aortic valve is a rare congenital anomaly often misdiagnosed as bicuspid aortic valve and typically presents with aortic stenosis rather than regurgitation.
Does valve-sparing aortic root replacement combined with bicuspidization improve valve function and left ventricular remodeling in a young adult with severe aortic regurgitation and aortic root dilation due to a unicuspid aortic valve?

Does valve-sparing aortic root replacement combined with bicuspidization improve valve function and left ventricular remodeling in a young adult with severe aortic regurgitation and aortic root dilation due to a unicuspid aortic valve?
This case demonstrates that transesophageal echocardiography is critical for distinguishing unicuspid from bicuspid aortic valves and that valve-sparing aortic root replacement with bicuspidization is a feasible option for young patients presenting with severe aortic regurgitation.
May support valve-sparing repair with bicuspidization in young unicuspid aortic valve patients; leaves open durability and generalizability.
BACKGROUND: Unicuspid aortic valve (UAV) is a rare congenital anomaly and is often misdiagnosed as a bicuspid aortic valve (BAV). It typically presents with aortic stenosis rather than regurgitation. CASE SUMMARY: A 21-year-old man presented with exertional dyspnea and was diagnosed with severe aortic regurgitation (AR) and aortic root dilation. He had been followed as having BAV since childhood. However, transesophageal echocardiography revealed a unicommissural UAV. The patient underwent valve-sparing aortic root replacement using the reimplantation technique combined with bicuspidization. Postoperative echocardiography showed mild residual AR with good valve function. DISCUSSION: UAV is frequently misdiagnosed on transthoracic echocardiography, and its presentation with severe AR is uncommon. This case highlights the diagnostic value of transesophageal echocardiography and the feasibility of valve-sparing surgery in selected young patients. TAKE-HOME MESSAGES: UAV may be overlooked as BAV and can present with severe AR. Accurate diagnosis and valve-sparing surgery are key in young patients.

Soejima et al. (2026) conducted a case report in Unicuspid aortic valve presenting with severe aortic regurgitation and aortic root dilation (n=1). Valve-sparing aortic root replacement using the reimplantation technique combined with bicuspidization was evaluated on Postoperative aortic regurgitation severity and left ventricular reverse remodeling. Valve-sparing aortic root replacement with bicuspidization reduced severe aortic regurgitation to mild, reduced LV end-diastolic volume from 228 to 170 mL, and improved LVEF from 62% to 67% at 6 months.
